Site Type:

Open sites are open to the community. Most often, sites operate as open sites.

Open Restricted sites are open to the community, but limited due to space, safety, security, or to maintain control. They CANNOT BE RESTRICTED to only a certain group of children (i.e. Summer school). A statement of reason and final approval to operate this type of site will be decided by the PA Department of Education.

Closed/enrolled sites that are NOT in "eligible areas" must also establish eligibility through the income eligibility of the individual children enrolled at the site. Sites must either collect household applications or receive verification of eligibility from the school each child attends. At least 50% of the enrolled children at the site must be eligible for free or reduced-price school meals to be able to participate as a site.

Most often, sites operate as "open." Please know that if you choose "closed", or "open restricted," you must provide a statement of reason and final approval to operate this type of site will be decided by the PA Department of Education.
(Required)
Please provide a statement indicating why your site will not be open to the community.

Describe your facility:

Do you have the capability to store refrigerated meals overnight?(Required)
In order to provide better service to our sites and reduce delivery errors that cannot be corrected, meals will be delivered the day before meal service (with few exceptions). Hot meals (in individual trays) will need to be heated prior to serving. Meals must be stored safely at the proper temperature until meal service.
Full-size refrigerators include both household and commercial grade refrigerators. Do not include apartment-size refrigerators in your total count.
Do you have a freezer available to store frozen meals?(Required)
We will provide sites with a supply of individual frozen meals as back-up meals in case of problems with scheduled deliveries and/or more children in attendance than anticipated.
Is there an indoor location at or near your site where boxes of back-up shelf-stable meals can be stored?(Required)
What equipment do you have at this site location for heating food?(Required)

Do you have a working sink available on site?(Required)
All sites sponsored by Greater Pittsburgh Community Food Bank will be using the Link2Feed electronic record-keeping system for counting and submitting meals served each day. The site can be accessed through internet browsers standard on any Apple or Android phone, tablet, or computer. Would site staff prefer to use their own smartphones or a tablet provided by the Food Bank?
The Link2Feed site does not require continuous WIFI access to work, but it does need to connect to WIFI to submit the information to us.
