Drive-Up Food Distribution Event - Sign Up

















The Food Bank’s records show that the vehicle you entered already has three reservations for this distribution. We are currently limiting reservations to three shares of food per car. The Food Bank has set this limit to ensure fairness and compassion, and to create efficiency. We appreciate your understanding.


There are no exceptions to three shares per car. For one car to receive two or three shares of food, the car must have more than one family or household. Each household must complete a separate reservation. The same car and license plate can be used up to three times for the same event. Please have the second household complete their own reservation.

Everyone can receive food and there is no required proof of eligibility. Your information will only be used for the purpose of providing you food and helping us improve your service. Your responses here will not affect the amount of food you receive.

The Food Bank encourages individuals to help their neighbors by acting as a proxy
for people who are homebound. While delivering food we encourage you to practice social distancing and avoid direct contact.

Household Size and Income                                              




If you recently lost your job or have been furloughed, you should not consider income from that job when answering this question. Examples of some other common sources of income to think about include are Social Security, pensions, or a part-time job.
Acknowledgment

To receive food from the Food Bank we ask people who make a certain amount of money or less based on household size digitally sign a "Self Declaration of Need". This signature helps us pay for and get the food we give you during food distribution events. If you answered the questions about household size and income correctly, please accept and sign the following statement. 

"I understand the household income limitations and hereby certify that my household size and income make me eligible for participation in the program. I also certify that, as of today, my household lives in the area served by Pennsylvania in The Emergency Food Assistance Program. This certification form is being completed in connection with the receipt of federal assistance."

"I understand that making a false statement may result in me having to pay for the value of the food improperly issued to me and may subject me to criminal prosecution under state and federal law."

"I have agreed to submit this Self Declaration of Need form by electronic means. I certify that my answers are correct and complete to the best of my knowledge. I understand that my typed signature has the same effect as a handwritten signature."
